Job summary

Sovereign & Bale is seeking two Health and Employment Specialists to provide health-led support to individuals facing barriers to employment in Leicester. You will deliver personalised coaching and develop support plans to enhance health, wellbeing and independence.

Joining a multidisciplinary team, you will collaborate with partners to help participants access health services, volunteering and job opportunities, using trauma-informed approaches and motivational interviewing where appropriate.

Qualifications

• Degree-level qualification (Level 6+) in health, wellbeing or behavioural science.
• Experience supporting individuals with health conditions, disabilities or complex barriers.
• Experience delivering person-centred coaching, rehab or behaviour-change interventions.
• Understanding of Social Model of Disability, safeguarding, boundaries and risk management.
• Strong communication and relationship-building skills, engaging anxious or low-confidence individuals.
• Experience maintaining accurate case records and outcomes.
• Knowledge of trauma-informed practice or health coaching advantageous.
• Experience in employability, vocational rehab or DWP-funded services desirable.
• Experience working in multidisciplinary teams with health, statutory or community organisations beneficial.

Responsibilities

• Deliver personalised one-to-one coaching and support to participants.
• Complete holistic assessments and develop personalised Support Plans.
• Help participants identify and overcome health, social and practical barriers.
• Build trusting relationships using person-centred and trauma-informed approaches.
• Support access to health services, community support, volunteering, learning and employment.
• Use coaching and behaviour-change techniques to promote progress.
• Monitor progress, measure outcomes and distance travelled.
• Collaborate with healthcare professionals, Jobcentre Plus and community organisations.
• Identify safeguarding concerns and manage risks appropriately.
• Maintain accurate case records and contractual documentation.
• Work effectively within a multidisciplinary team.
