Job summary

Sovereign & Bale in Staffordshire is seeking two Health and Employment Specialists to provide personalised, health-led support to individuals facing barriers to employment and independence. You will offer person-centred coaching to help participants improve health, wellbeing, confidence and independence while progressing towards goals at a pace that suits them.

You will work within a multidisciplinary team to identify barriers and develop practical support plans.

Qualifications

• Degree-level qualification preferred in health, wellbeing or behavioural science.
• Experience with person-centred coaching and safeguarding.
• Ability to build trust and engage individuals who may be anxious or disengaged.
• Experience working in multidisciplinary settings with healthcare or community organisations.

Responsibilities

• Deliver personalised one-to-one coaching and support to participants.
• Complete holistic assessments and develop personalised Support Plans.
• Help participants identify and overcome health, social and practical barriers.
• Build positive, trusting relationships using person-centred and trauma-informed approaches.
• Support participants to access health services, community support, volunteering, learning and employment opportunities.
• Use coaching and behaviour-change techniques to encourage progress.
• Monitor participant progress and measure outcomes and distance travelled.
• Work collaboratively with healthcare professionals, Jobcentre Plus, community organisations and employment services.
• Identify and manage safeguarding concerns and risks.
• Maintain accurate case records, outcome measures and contractual documentation.
• Work effectively as part of a multidisciplinary team.
