Midlands Partnership NHS Foundation Trust is seeking a Health & Justice Support Time Recovery worker for the Black Country region. The role focuses on short-term support for individuals leaving custody or court, identifying needs, and coordinating referrals to appropriate services.

The post involves liaising with multi-disciplinary teams, completing risk assessments, and delivering interim care within a dynamic Health & Justice service based at Crescent House, Bilston.

Qualifications

• Competent in literacy and numeracy.
• Basic knowledge of physical/mental condition and rehabilitation.
• Computer literacy.
• NVQ Level 2 Health & Social Care or equivalent.
• Adult/Child safeguarding training.

Responsabilités

• Communicate sensitive information with patients, utilising highly developed communication skills to overcome barriers to understanding.
• Establish therapeutic relationships with service users and relatives/carers.
• Participate in setting quality standards and auditing practice.
• Assess patient care needs and refer to appropriate services; involve the individual in planning.
• Complete and contribute to risk assessments and risk management plans; promote positive risk taking.
• Maintain an effective reporting system on patient conditions.
• Utilise outcome measures to review service effectiveness and contribute to service development.
• Support clinical governance processes at team level.
• Service is based at Crescent House, Bilston, with community support across the Black Country.
