This role focuses on ageing well and proactive care in care homes, delivering education, training and evidence-based interventions to promote physical activity, falls prevention and wellbeing.

Qualifications

• Health improvement facilitator focusing on ageing well and proactive care in care homes.
• Deliver structured programs of support and proactive care to care staff and residents.
• Work with multidisciplinary colleagues to embed preventative approaches and improve outcomes.

Responsibilities

• Support the delivery of education, training and evidence-based interventions to promote physical activity, falls prevention, healthy ageing and wellbeing for people living in care homes.
• Deliver group education sessions across topics including physical activity and supporting quality improvement initiatives to promote best practice in ageing well.
• Collaborate with multidisciplinary colleagues, care home providers and partner organisations to improve outcomes for residents by encouraging person-centred care and maintaining independence.
• Contribute to service development through data collection, audit, evaluation and the promotion of evidence-based practice.
