Sirona care & health, England's largest provider of Adults and Children's Community Services, invites applications for Nurse Associate or Assistant Practitioner roles in Bristol, North Somerset or South Gloucestershire. You will work with a multi-disciplinary team on a 8am-8pm rota over 7 days, using electronic records and prioritising workload.

We offer Agenda for Change pay, full NHS benefits, 27+ days leave and NHS Pension.

Qualificações

• Experience in health or social care and direct patient care.
• NMC registration and competence in nursing practice.
• Willingness to undertake relevant training.

Responsabilidades

• Undertake delegated holistic assessments and plan care for acutely/chronically ill patients.
• Assess and monitor health needs and patient well-being.
• Collaborate with multi-disciplinary team to set goals and referrals.
• Support and educate service users and carers to promote independence.
• Maintain accurate records and participate in training and development.
