Job summary

Qualifications

• At least three years of direct service experience in Health Home or related chronic disease management.
• Bachelor’s degree required; higher degrees may substitute experience as noted.
• At least 1 year of supervisory/management experience.

Responsibilities

• Provide direct supervision to Care Managers for interventions and performance.
• Track client engagement and program performance.
• Ensure coverage of Care Manager caseload and provide case management when needed.
• Review and approve client documentation per program standards.
• Oversee enrollment, eligibility, and case assignment processes.
• Provide crisis intervention to distressed clients.
• Review data reports to monitor compliance and performance.
• Lead staff meetings, supervision, and training.
