We are looking to recruit Health Care Support Workers to join either our Celandine Health & Metrocare PCN based in Ruislip or our Synergy PCN based in Uxbridge.
Main duties of the job
A HealthcareSupport Worker will work within amulti-professional team in a GP setting, providing, support, care, andtreatment to patients, within guidelines and protocols identified for the role.

Job responsibilities
- Supportshealth promotion and prevention activities across the PCN, including smokingcessation, weight management, and lifestyle interventions
- Playsa key role in vaccination and immunisation programmes within the PCN
- Workswithin a Multi Disciplinary Team to support patients with long-term conditionsand health inequalities;
- Providesclinical support to registered nurses and other healthcare professionals withinthe PCN;
- Deliversbasic clinical care interventions under supervision, supporting continuity ofcare and patient safety initiatives.
- Takingand recording Blood Pressure, height & weight
- PerformingECGs and Phlebotomy
- Assistingwith chronic disease monitoring
- AssistingGPs and nursed with minor procedures
- Updatingpatient records and ensuring accurate documentation
Personal/Professional Development:
- Post holder must hold a carecertificate or work towards one
- Participation in an annual individualperformance review, including taking responsibility for maintaining a record ofown personal and/or professional development
- Taking responsibility for owndevelopment, learning and performance and demonstrating skills and activitiesto others who are undertaking similar work
- Inform the lead GP of any concernsregarding role and any professional development needed
- Be aware of own professionalboundaries and what to do when you have reached them

Person Specification
Qualifications
- GCSE Grade A to C in English and Maths
- Has or willing to work towards a care certificate or equivalent qualification

- Full Driving Licence
Experience
- Experience of coordinating tasks and liaising with multiple healthcare professionals both internal and external to the organisation.
- Experience of using technology and digital tools
- Primary Care experience as a Healthcare Assistant, Practice Administrator, or other.
- Experience of working with healthcare professionals and or previous experience in the NHS or social care or relevant field.
- Experience of using clinical systems such as EMIS.
