Phyllis Tuckwell is looking for dynamic andmotivated Health Care Assistants to join our team on the In-Patient Unit at theHospice. As we are currently in the process of building a new Hospice on ourexisting site in Farnham, our In-Patient Unit (IPU) - and this role will bebased on a temporary basis at Kings Lodge Care Home in Camberley. We are aimingto move into our new Hospice within 12months, at which point our IPU -and this role will relocate from Kings Lodge in Camberley to the new Hospice inFarnham. The Hospice Care at Home teams are based in Farnham and Guildford.

Main duties of the job

This is an exciting opportunity for an enthusiasticHealth Care Assistant who is looking for a rewarding role where they are ableto provide high quality patient & family focused care within a supportiveenvironment. Successful candidates will have previous experience in acaring role, has/will undertake a level 2 related care qualification, and willpossess excellent communication, interpersonal and team working skills. Whilstpalliative care experience is valuable, it is not essential, as full training andsupport will be given.

Job Purpose:Under the direction and supervision of qualified registered staff, to maintain andsupport the highest standards of nursing care, so as to ensure that a high quality andcomprehensive service is provided to all patients, their families and carers.

Responsibilities:

1.0 Clinical

1.1 To ensure statutory obligations are met that are relevant to the clinical area.

1.2 To ensure quality standards are maintained in order toprovide the higheststandards of holistic care.

1.3 To maintain and support the provision of assessment,planning, implementation, and evaluation of nursing care,within the limits of your qualifications and under thesupervision of qualified staff.

1.4 Work to the agreed models of care and work allocation.

1.5 Escalate any clinical or non-clinical incidents to the nurse incharge, and complete incident documentation via Sentinel.Assist with investigations and risk assessments asappropriate.

2.0 Professional

2.1 To understand and work within the Health and Social Care Act 2008, and PTbusiness plan and philosophy.

2.2 Work as part of the multidisciplinary team, providing holistic care topatients, their families and carers.

2.3 Deliver a safe and high standard of care in accordance with PhyllisTuckwell policies,guidelines.

2.4 Where appropriate and under the supervision of qualified staff,provide information and support to patients, carers and relatives.

2.5 Maintain effective relationships with colleagues.

2.6 To maintain accurate, legible and contemporaneous records on allpatients, to ensure documentation meets agreed PT standards andadheres to Caldicott Guidelines.

3.0 Managerial

3.1 To work in accordance with the PT, Health and Safety Governance policies.

3.2 To effectively maintain and support the clinical area and monitorresources including clinical supplies and equipment.

3.3 To ensure a team environment is maintained by developing effectiverelationships and being considerate towards colleagues, to create ateam spirit and provide personal support.

3.4 Manage your own time effectively to meet PT aims and objectives.

4.0 Training and DevelopmentTeam

4.1 Maintain and support the professional development and safe practice of othersthrough peer support, leadership, supervision and teaching.
