Job summary

Essex Partnership University NHS Foundation Trust seeks a Care Assistant to support the senior team in providing care and physical rehabilitation for patients needing short-term recovery at home after an acute event. The role aims to help patients achieve maximum independence and live as safely as possible.

You will work with therapists to deliver direct care and rehabilitation, encourage activities of daily living, monitor health, and contribute to care planning and risk management.

Responsibilities

• Work as part of the multidisciplinary team and, when necessary, on your own initiative.
• Provide direct care and rehabilitation to the patients served by the team.
• Protect and maintain patients' dignity and respect, observing religious and cultural practices.
• Encourage patients to participate in activities of daily living and engage in therapeutic activities.
• Assist patients to work towards rehabilitation goals to enhance independence and wellbeing.
• Participate in manual handling of patients per the Trust’s Manual Handling Policy.
• Follow Infection Control Policy and wear appropriate PPE.
• Observe, record and report changes in patients' health and environment to senior staff.
• Be familiar with steps in medical emergencies and act accordingly.
• Complete observations including urinalysis and blood sugar monitoring as required.
• Support the senior team during prelim, first, and review visits to patients.
• Collaborate with therapists to support effective assessment and treatment, including in-patient beds if needed.
• Assist in planning work with the senior team and contribute to care planning and risk management.

Job Overview

To assist the senior team in providing care and physical rehabilitation to people who require short team period of recovery in their own homes following an acute event or deterioration that may or may not have resulted in an acute hospital stay. The aim is to provide person centred rehabilitation to enable patients to reach their maximum potential and live as independently as possible.

Main duties of the job

• To work as part of the multidisciplinary team and when necessary on own initiative.
• To provide direct care and rehabilitation to the patients served by the team.
• To act in a manner that protects and maintains the patient’s sense of dignity and respect, while observing their individual religious and cultural practices.
• Encourage patients to participate in activities of daily living and facilitate their active involvement in therapeutic activities.
• Assist the patient work towards to rehabilitation goals to enhance independence and wellbeing.
• Ensure active involvement in manual handling of patients as required – in line with the Trust’s Manual Handling Policy.
• To work within the guidance of the Infection Control Policy and wearing appropriate protective clothing at all times.
• Observe record and report changes in the patients’ health and environment to senior staff.
• To be familiar with the steps to be taken in medical emergencies and act accordingly.
• Complete observations including urinalysis and blood sugar monitoring as required.
• Assist the senior team with prelim visits, first visits and review visits to patients.
• Work directly with the therapists within the team to support them providing effective assessment and treatment including in the in-patient beds if necessary.
• Assist in planning work together with the senior team.
• Assist in contributing to care planning and risk management plans.
