Job summary

The Haematology Clinical Nurse Specialist role at Wye Valley NHS Trust joins the outpatient haematology team to support patients with malignant or non-malignant blood conditions. You will act as a key worker, deliver nurse-led reviews, monitor treatment and liaise with primary and tertiary services.

The post requires NMC registration and proven experience in haematology, CPD, and advanced clinical skills. Travel between sites is involved and you will support patients and families while

Qualifications

• NMC registration as a registered nurse.
• Experience sufficient to understand disease management options and side effects of haematological treatments.
• Evidence of continuing professional development.
• Demonstrated delivery of advanced clinical skills in practice.

Responsibilities

• Work as a key worker for patients with malignant or non-malignant haematological conditions.
• Act as a core member of the haematology team in line with Improving Outcomes Guidance for haematological malignancies.
• Undertake nurse-led reviews of in-patients.
• Take part in MDT meetings, in-patient reviews, consultant outpatient clinics, bone marrow biopsies, nurse-led clinics and treatment monitoring reviews.
• Liaise with Macmillan Renton Unit staff on investigation, treatment and follow-up.
• Provide level 2 psychological support and refer patients to a Clinical Psychologist when appropriate.
• Coordinate treatment delivery across shared care pathways with primary and tertiary care.
• Give specialist advice and education to other Trust health professionals.
• Support patients, carers and families.
• Develop and maintain varied formats of patient information.
• Support the development of nurse-led clinic activity.
