Job summary

Procare Health Limited is seeking an experienced GP with Extended Role for the Haslemere Proactive Care Hub. You will work 4-6 sessions per week within a multidisciplinary team to support frailty and complex long-term conditions, coordinating care beyond single conditions.

The role emphasizes proactive, integrated care with community services, using CGA and personalised anticipatory plans alongside diverse clinicians.

Qualifications

• GMC registered medical practitioner is required.
• Interest in frailty, community geriatrics, proactive care or complex multimorbidity.

Responsibilities

• Undertake comprehensive assessments of frailty and complex needs.
• Provide GP clinical review and sense-checking of specialist recommendations.
• Support medication optimisation and develop personalised anticipatory care plans.
• Collaborate with MDT across primary, community and specialist care.
