Job summary

Birmingham Community Healthcare NHS Foundation Trust seeks an experienced Practice Pharmacist to support safe and cost‑effective use of medicines within the practice. Responsibilities include repeat prescribing, medicines reconciliation after discharge, and medicine reviews to optimise treatment and adherence.

You will respond to prescription queries, monitor high‑risk medicines, advise patients and carers, and work with GPs and hospital teams to ensure continuity of care.

Qualifications

Maintain GPhC registration and undertake CPD.
Experience in community or practice pharmacy is desirable.

Responsibilities

Support safe, cost-efficient use of medicines across the practice.
Process and manage repeat prescription requests.
Support medicines reconciliation after hospital discharge.
Assist with medication reviews and adherence.
Respond to routine prescription queries.
Identify and escalate medication-related risks.
Communicate with patients and carers about medicines.
Undertake searches, audits and reviews for patient monitoring.
Reduce medicines waste and improve prescribing efficiency.
Collaborate with GPs, pharmacists and hospital teams.
Maintain accurate, confidential patient records.
