Job summary

St Johns House Medical Centre is seeking a Salaried GP to join a young clinical team serving a list of around 14,200 patients. The role focuses on surgery, telephone and home-visit consultations with autonomous clinical decision-making and care planning.

You will develop care plans, prescribe per practice standards, and lead on QOF domains while benefiting from mentorship for portfolio and leadership roles. Training support and CPD are offered within a supportive environment.

Qualifications

• GMC registration as a GP.
• Membership of an approved medical defence organisation.
• Entry on the Performers List.
• DBS clearance

Responsibilities

• Carry out surgery consultations, telephone consultations and home visits in line with the practice timetable.
• Make autonomous clinical decisions on presenting problems, whether self-referred or referred by other staff.
• Assess undifferentiated and undiagnosed health needs and screen for disease risk factors and early signs of illness.
• Develop care plans in line with current disease management protocols and provide counselling and health education.
• Compile and issue computer-generated prescriptions, check and sign repeat prescriptions, and prescribe per practice standards and guidelines.
• Admit and discharge patients to and from the caseload and refer to other care providers as appropriate.
• Record clear, contemporaneous consultation notes, complete clinical templates and contribute to patient record summaries and Read-coding.
• Deal promptly with test results, medical reports, referral letters, paperwork and electronic correspondence.
• Lead on one or more clinical QOF domains as agreed with the partners and practice manager.
• Collect and use data for audit and contribute to evaluation and clinical standard setting.
• Meet deadlines for audits and written returns to secure QOF, LES and DES funding.
• Keep up with policies and guidelines and attend practice and external training and meetings
