Job summary

Suffolk GP Federation CIC is seeking enthusiastic General Practitioners to join our Care Management Service, providing clinical leadership within multidisciplinary neighbourhood teams and helping deliver proactive care to patients with complex needs.

You will collaborate with clinicians, care coordinators, community teams and voluntary sector partners to identify opportunities to improve outcomes and reduce unnecessary hospital admissions, helping shape innovative, integrated care across Suffolk.

Qualifications

• Local GP with GMC Registration.
• You will need to be a local GP and be aware of local pathways.

Responsibilities

• Provide clinical leadership and oversight within multidisciplinary team meetings.
• Support the identification, assessment and management of patients at high risk of unplanned healthcare utilisation.
• Contribute to the development, review and optimisation of personalised care and support plans.
• Join the Care Management Service to deliver proactive, person-centred care across Suffolk.
