Job summary

Change Grow Live in the United Kingdom is seeking a Through the Gate Worker at HMP Channings Wood to support people approaching release, helping them access treatment, healthcare, and community services while they rebuild lives. You will work 37.5 hours per week, with pro rata for part-time roles.

Experience in substance misuse, criminal justice, mental health, or social care is beneficial but not essential, and full training will be provided.

Responsibilities

• Support individuals during the 12 weeks leading up to release.
• Develop personalised resettlement and recovery plans.
• Coordinate referrals to healthcare, treatment, and community services.
• Work closely with prison staff, healthcare teams, resettlement teams and external partner agencies.
• Advocate for individuals to access the support they need.
• Promote recovery, resilience, social reintegration, and independence.
• Contribute to risk management and safeguarding processes where required.

Experience in substance misuse, criminal justice, mental health, social care, or a related field can be beneficial, but it isn't essential. Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
