Responsabilidades

• Deliver face-to-face outpatient clinics across community setting
• Undertake home visits, supporting patients to achieve their goals in their own environment
• Manage a varied and engaging caseload across Medway and Swaleo Work collaboratively with multidisciplinary teams to deliver outstanding patient care
• Plan and deliver patient education groups that empower individuals to improve their health
• Contribute to service development initiatives and quality improvement projects that shape the future of care
• Be supported in your professional development and specialist interests, including Gastroenterology
