Kent Community Health NHS Foundation Trust is seeking a Band 6 Dietitian to support the Adult Community Dietetics Team. The role involves delivering outpatient clinics in community settings, providing nutrition support, and delivering education groups.

A flexible arrangement is considered, with a driving licence required for home visits and travel across Medway and Swale. The post emphasizes gastroenterology interest and multidisciplinary collaboration to improve patient outcomes.

Formación

• Knowledge of a range of therapeutic interventions for the client group and their relative benefits
• Evidence of continued professional development applied in practice within the last 12 months

Responsabilidades

• Deliver face-to-face outpatient clinics across community settings
• Undertake home visits to support patients in their own environment
• Manage a varied caseload across Medway and Swale
• Provide nutritional support and plan and deliver patient education groups
• Work with multidisciplinary teams to deliver patient care
• Contribute to service development and quality improvement projects
• Travel across the service area, which requires a driving licence and vehicle access
