Job summary

Transforming Support is seeking an experienced Support Worker to join TS Liverpool’s service at Laystall Street, Manchester. You will provide person-centred support to help people build confidence, independence and access activities in the community.

You will work full-time on a rolling two-week rota, with full training, a funded DBS check and ongoing development. Bank holidays and leave are included in the benefits.

Qualifications

• Six months+ experience in a care or support setting.
• Available to work full time on the rolling rota.
• Able to travel to Laystall Street, Manchester, M1 for shifts.
• Communicate clearly and maintain accurate written support records.
• Right to work in the UK for full contracted hours without sponsorship.

Responsibilities

• Support everyday living skills and independence for people.
• Help with routines, activities and personal goals.
• Assist access to community, appointments and social activities.
• Provide support with health needs and medication per procedures.
• Follow individual support plans and contribute to reviews and risk management.
• Maintain accurate records and collaborate with families and professionals.
