Job summary

HCRG Care Group invites applications for a Community Support Worker at Melksham Community Hospital. The role focuses on delivering care within a community setting, under supervision, and contributing to patient-centred plans.

You’ll work within a multi-disciplinary team and may undertake nursing or therapy activities as skills progress. The post is full-time, 37.5 hours per week, with a salary in the £25,500–£27,098 range, and benefits including a group pension and wellbeing support.

Qualifications

• Good general education (English/Maths Grade A-C) required.
• Level 2 NVQ/QCF in Health and Social Care or equivalent.
• Care Certificate – or willingness to achieve.
• Experience of working under supervision.
• Experience of supporting other workers.
• Ability to work as a team member.
• Ability to deliver patient focused care.
• Full UK driving licence and use of a car during working hours.
• Experience in community health care, rehabilitation and urgent care settings.

Responsibilities

• Deliver care within a community setting per written care plan.
• Keep patient at centre of care with patient and carer participation.
• Support day-to-day care including monitoring and communication.
• Work in a multi-professional team and feed back progress.
