Job summary

Countess of Chester Hospital NHS Foundation Trust is seeking an Advanced Clinical Practitioner (ACP) to join the Frailty team managing the SDEC Frailty Unit. The ACP will work autonomously delivering clinical care, providing expert advice and leadership to ensure high-quality patient outcomes.

The successful applicant will make and maintain clinical decisions and treatment plans to manage acute illnesses, working within a multidisciplinary team to deliver efficient, compassionate care across

Qualifications

• Autonomous clinical decision making within the Frailty Unit.
• Leadership responsibilities within the Frailty team.
• Multidisciplinary collaboration to ensure high-quality patient care.
• Experience as Advanced Clinical Practitioner (ACP) preferred.
