Job summary

The Countess of Chester Hospital NHS Foundation Trust is recruiting for a permanent Advanced Clinical Practitioner to work within the Same Day Emergency Care (SDEC) and Emergency Department footprint. This full-time role requires autonomous clinical care, expertise and support to facilitate same day discharge and prevent unnecessary admissions.

The ACP will collaborate with a multidisciplinary team while also practising independently, with a background in frailty being advantageous.

Qualifications

• MSc Advanced Practice/Clinical Masters with evidence of clinical competencies sign off.
• AHP or Nursing Diploma or Degree HCPC/NMC Registered.
• CPD portfolio and relevant clinical skills to support clinical practice.

Responsibilities

• Provide autonomous clinical care and advice within SDEC/ED to support same day discharge.
• Work with multidisciplinary team to optimise patient flow and reduce unnecessary admissions.
• Deliver expert clinical guidance and support for frailty-related cases when advantageous.
