Job summary

Frimley Health NHS Foundation Trust is seeking a Band 5 Staff Nurse to join the Frailty Same Day Emergency Care (SDEC) team. You will provide rapid, person-centred nursing assessments for older adults with frailty, coordinating care with geriatricians, therapists and community colleagues to support safe discharge.

The role requires current NMC registration, ability to work autonomously in a fast-paced environment and excellent communication and teamwork skills.

Qualifications

• Must be a Registered Nurse (NMC registered) with clinical experience in emergency care or SDEC.
• Able to work autonomously and within a multidisciplinary team.
• Excellent communication, teamwork and organisational skills.
• Experience with frailty and geriatric assessments is desirable.
• Committed to evidence-based, person-centred care and service improvement.

Responsibilities

• The assessment, planning, implementation, and evaluation of nursing care
• Supporting shift coordination to ensure safe, efficient, and effective care delivery
• Collaborating with multidisciplinary colleagues including geriatricians, GPs, physiotherapists, and pharmacists
• Acting as a key link between primary, secondary and social care to reduce unnecessary admissions and support safe discharge planning
• Help promote admission avoidance, reduce hospital length of stay, and support early intervention for individuals living with frailty.

Job description

An exciting opportunity has arisen for an enthusiastic and compassionate Band 5 Staff Nurse to join our Frailty Same Day Emergency Care (SDEC) team. You will work within a highly skilled multidisciplinary service providing rapid, holistic assessment and person-centred care for older adults living with frailty, supporting timely treatment and safe discharge or transfer. This is an excellent opportunity to develop specialist expertise in acute frailty, rapid assessment, clinical decision-making, medication management and complex discharge planning. Working closely with advanced practitioners, geriatricians, therapists, support staff and community colleagues, you will assess, plan, implement and evaluate individualised programmes of care, respond promptly to changes in patients' needs and help maintain safe and efficient patient flow through Frailty SDEC. We are looking for a motivated, adaptable registered nurse who can prioritise safely, work effectively both independently and as part of a team, and remain calm in a fast-paced clinical environment. As a key member of theFrailty SDEC team, you'll provide high-quality nursing care to older adults presenting with frailty syndromes. You'll work closely with Emergency Department colleagues to proactively identify and manage patients, contributing to comprehensive geriatric assessments and the creation of supportive management plans. You will be responsible for:
