Job summary

Gateshead Health NHS Foundation Trust is seeking an experienced Frailty Nurse Practitioner (Band 7) to join the Frailty Service. You will work across the Acute Frailty Team and Frailty Virtual Ward, delivering advanced assessment, decision-making and person-centred care for patients living with frailty in both acute and community settings.

Responsibilities

• Work autonomously as Frailty Nurse Practitioner providing high-quality, person-centred care.
• Hold a defined caseload of patients with complex needs including frailty and multi-morbidity.
• Undertake comprehensive assessment, care planning, intervention and evaluation.
• Provide specialist clinical advice to patients, carers and MDT members.
• Develop and oversee complex caseload management across acute and community settings.
• Promote continuity of care and safe transitions between care settings.
