• Post graduate master’s level Qualification in Advanced Practice or equivalent.
• Evidence of development in self/others/service.

Responsibilities

• Perform Comprehensive Geriatric assessment to screen frailty and plan multidisciplinary care.
• Prioritise care and provide daily medical reviews for frail patients with the MDT.
• Communicate clearly with patients and carers about conditions and anticipatory care planning.
• Work across organisational boundaries with health and social care teams.
