Job summary

Weymouth and Portland PCN is seeking a Frailty Nurse to join the team delivering proactive care to frail patients in the community, including housebound individuals.

The role involves home visits, baseline assessments, ECG and phlebotomy, and coordinating with a multi-disciplinary team to support patients and reduce hospital admissions. Experience in elderly care is valued, with a focus on holistic, patient-centred care.

Qualifications

• Hold Full Registration with the NMC plus Licence to Practise.
• Current UK professional registration is required.

Responsibilities

• Conduct home visits to assess baseline needs.
• Perform ECG and phlebotomy as part of assessments.
• Make referrals to MDT as appropriate.
• Contribute to Dorset Care Plan and data collection.
• Maintain comprehensive record keeping and documentation.
