Kernow Health in Cornwall is seeking to join our Integrated Frailty Team across 3 Harbours and Bosvena INT. You will work alongside our Nurse Consultant, care coordinators, clinicians, Pharmacy Technician, OT, and community nursing teams to support frail, long-term or acutely housebound patients and nursing/residential homes within our Integrated Neighbourhood.

With knowledge in frailty, physical assessment and clinical reasoning, you will use these skills to assist in recognizing early symptoms

Qualifications

• Knowledge and skills in frailty, physical assessment and clinical reasoning.
• Ability to assess, diagnose, plan, organise and review complex packages of care.

Responsabilités

• Respond to urgent referrals for patients, carers and the MDT.
• Case manage and coordinate with adult social care for care packages.
• Work within the framework of an MDT.
• Assess, diagnose, plan, organise and review complex care packages, including conferences.
• Provide holistic evidence-based care in line with policies and individual care plans.
• Ensure privacy, dignity and human rights for patients and carers.
• Undertake comprehensive risk assessments to ensure safety.
• Collaborate with Occupational Health on assessments and equipment use.
• Record and report incidents per policy; maintain confidential records.
• Oversee patient records quality by audit and peer review; discuss with team.
• Comply with infection control policies and attend updates; report concerns.
• Consider safeguarding per organisational policy.
