Job summary

Norfolk and Norwich University Hospitals NHS Foundation Trust is seeking an experienced Health Care Assistant to join the Frailty SDEC/Loddon nursing team at the Colney Lane site. The role focuses on caring for elderly patients through the frailty pathway, with support from a multidisciplinary team.

Qualifications

• Delivery of clinical and therapeutic care to patients within set protocols.
• Plan and deliver high standards of individualized personal care per Essence of Care and Trust policies.
• Effective communication within the team, with other healthcare professionals and with patients, families and carers.

Responsibilities

• Carry out instructions from registered MDT members who are clinically accountable.
• Under delegation, perform clinical skills appropriate for the role (e.g., wound care, taking blood and ECG, vitals).
• Work a minimum of 36 hours per week.
• Join the Frailty SDEC/Loddon nursing team at Norfolk and Norwich University Hospitals NHS Foundation Trust.
