Job summary

Harrow East Primary Care Network is seeking a Frailty Care Coordinator to support its frailty service across Harrow and Stanmore sites. You will coordinate appointments, referrals and MDT meetings, maintain records and ensure timely, person-centred care for patients living with frailty.

Working closely with the Frailty GP and wider team, you will manage administrative tasks, monitor caseloads and support service development.

Responsibilities

• Frailty caseload coordination with dedicated administrative and care coordination support.
• Arrange and coordinate patient appointments, home visits and follow-up reviews.
• Organise frailty MDT meetings, prepare agendas and collate relevant information.
• Serve as a point of contact for administrative queries and liaise with patients and carers.
• Maintain patient records using EMIS Web and other systems; support information governance.
• Contribute to ongoing development and improvement of the frailty service.
