Job summary

Vertis Health is seeking a compassionate Care Coordinator to join the Frailty Team at Worcester City PCN. The role involves supporting patients living with frailty and complex health needs, coordinating care across health, social care and voluntary services, and delivering person-centred care planning.

Working with GPs, community teams and partner organisations, you will help patients access the right services, book appointments, and support independence through timely, joined-up care.

Qualifications

• Compassionate and proactive care coordinator
• Experience supporting patients with frailty and complex health needs
• Strong ability to coordinate across health and social care services

Responsibilities

• Coordinate care across health, social care and voluntary services for frail patients
• Develop personalised care plans with patients and carers
• Support access to services, appointments and information
• Work with GPs, community teams and partner organisations
• Occasional home visits and visits to multiple PCN practices
