Job summary

Qualifications

• Post qualifying experience as a social worker.
• Autonomy in a multi-disciplinary team.
• Experience of working with people with complex mental health needs.
• Experience of working with people with learning disabilities and/or autism.
• Ongoing CPD and professional development.
• Experience of working with people in forensic services.
• Experience with adults with substance misuse or dual diagnosis.
• Demonstrate leadership for the social work profession in an MDT setting.

Responsibilities

• Maintain caseloads and provide care co-ordination and social supervision.
• Develop and monitor step down/discharge pathways.
• Provide consultation, support and guidance to colleagues and service users in the community.
• Work with Community Care Coordinators to facilitate transfers and liaise with health, commissioning and other stakeholders.
• Ensure timely, personalised pathways aligned with recovery goals and minimal restrictions.
