Job summary

Avon and Wiltshire Mental Health Partnership NHS Trust is recruiting a Specialist Community Forensic Worker to join a dynamic MDT. You will coordinate care for service users transitioning from secure care to the community and provide specialist advice to professionals.

The role involves care planning, risk assessments, producing clinical reports for tribunals and the MOJ, and training opportunities to develop in DBT, TIC and other tools within an innovative service.

Qualifications

• Minimum 2 years post-qualification mental health experience.
• Ability to coordinate care with an MDT.
• Willingness to train as social supervisor.
• Experience producing clinical reports for tribunal and MOJ.
• Skilled in risk assessment and care planning.

Responsibilities

• Care coordination for service users transitioning from secure care to the community.
