Aufgaben

• Proactively address delayed transfers of care across the service.
• Provide transfers expertise and knowledge, regarding pathway issues internally and externally, including delivering training to share knowledge.
• Achieve and report key performance measures, such as a 10% decrease in delayed discharge days.
• Identify and challenge barriers to care progression.
• Monitor and review failed transfer of care plans; plan reviews and devise alternative strategies.
• Identify patients approaching discharge and support ward teams to overcome barriers, potentially attending ward rounds.
• Attend meetings such as Weekly Referrals, CRFD Meeting, and Monthly Pathway Management Meeting.
• Act as a point of contact between internal teams and external agencies.
• Keep patients informed about transfer of care or discharge progress.
