Job summary

Sussex Partnership NHS Foundation Trust is seeking a Band 7 mental health specialist for the Forensic Outreach and Liaison Service (FOLS) to work with out-of-area patients and plan discharge from hospital to the community.

You will conduct specialist assessments, identify care pathways, and lead on service evaluation and quality improvement, working closely with the MDT and provider case managers. Regular supervision and development days are provided, with travel across the county and beyond.

Qualifications

• Qualified mental health professional (RMN/OT/Social Worker) required.
• Experience in discharge planning and community mental health recommended.
• Band 7 level with leadership in service evaluation and MDT work.

Responsibilities

• Carry out specialist assessments and identify pathways of care for each patient.
• Lead on service evaluation and quality improvement.
• Provide regular supervision and participate in professional development days.
• Collaborate with FOLS and provider case managers to plan discharge pathways.
• Monitor progress and CPA reviews with service users, carers and providers.
• Travel across the county and beyond as needed.
