Job summary

North London NHS Foundation Trust in London is seeking an Occupational Therapist to work on discharge pathways and in the community across Barnet, Enfield, Haringey, Camden and Islington. You will assess daily living skills, support risk management and promote healthy routines.

You will work within a large OT team, delivering 1:1 sessions and group programmes, contributing to MDT care planning and engaging with multidisciplinary colleagues to ensure safe, effective discharge and ongoing

Qualifications

• Qualified Occupational Therapist with valid registration.
• Experience with discharge planning and community-based care.
• Interest in mental health and forensic settings.

Responsibilities

• Providing OT on discharge pathway and community-based care.
• Supporting discharge needs across Barnet, Enfield, Haringey, Camden and Islington.
• Assessing and intervening for patients detained under The Mental Health Act.
• Delivering 1:1 sessions and group programmes with a Technical Instructor.
• Contributing to MDT care planning and risk assessment, verbally and in writing.
• liaising with internal and external services to support discharge.
