Job Description:

The dedicated team in Camden is seeking an experienced Community Psychiatric Nurse with CHMT experience to deliver high-quality integrated health and social care for individuals facing mental health issues. You will work closely with service users to provide personalized support tailored to their needs.

Your responsibilities will include, but are not limited to:
  • Maintaining high standards of care while respecting the individuality and dignity of service users, promoting independence.
  • Conducting individual assessments and updating care plans in collaboration with service users, in line with the Trust’s CPA Care Coordination Policy.
  • Communicating information regarding service users’ needs and progress promptly and accurately, respecting confidentiality.
  • Applying a Behavioural Family Therapy approach where appropriate.
  • Facilitating open communication within the team and with other agencies to promote multidisciplinary care.
  • Working as a CPA Care Coordinator for service users, if required.
  • Encouraging positive mental health practices among service users.
  • Providing psychiatric nursing interventions and psychological advice.
  • Participating in regular duty rotations.
  • Ensuring a recovery-focused approach to patient interventions.
Requirements:
  • Minimum of 6 months experience in a similar role.
  • Experience with CMHT.
  • Degree relevant to NMC registration and current NMC registration.
  • Experience in taking responsibility and supporting practice development.
  • Enhanced DBS check (preferably via the update service). Fast-track options available.
  • Mandatory training certificates.
  • Full driving license and access to a personal vehicle are advantageous.
