Job summary

South West Yorkshire Partnership NHS Trust in Wakefield is seeking a part-time Community Occupational Therapist to provide specialist OT assessments and develop care plans for service users transitioning from secure settings to the community.

You will work with a multi-disciplinary team, deliver individual and group OT interventions, and engage carers and families to support social participation and community living.

Qualifications

• Specialist knowledge of occupational therapy assessments and interventions in mental health.
• Experience delivering individual and group OT treatment in community settings.
• Ability to work autonomously and as part of a MDT.
• Full driving licence and access to a car preferred.

Responsibilities

• Undertake specialist OT assessments and risk assessments.
• Develop and deliver individualized OT plans of care.
• Lead case management for service users.
• Collaborate with agencies to support transition to community living.
• Maintain clinical records and participate in supervision.
