Job summary

Codi Group in Newport is seeking a dedicated Floating Support Worker to join the Pathways Floating Support Team. This full-time role involves delivering person-centred support to individuals with challenging histories, including substance use concerns.

You will manage caseloads, travel to clients’ homes or agreed venues, and work with partner services to ensure tailored, coordinated support that helps people achieve positive outcomes.

Qualifications

• Passion to work with and enable people to live as independent a life as possible.
• Ability to work alone as well as in a team.
• Ability to handle conflicting and difficult situations.
• Understanding of homelessness and complex needs.

Responsibilities

• Manage your own caseload and electronic diary.
• Follow safe working practices, including Lone working policy.
• Travel to clients' homes or agreed neutral venues.
• Engage with clients to establish goals and support plans.
• Provide guidance on finances, health, welfare benefits, housing options, digital inclusion and tenancy skills.
• Collaborate with external agencies and signpost to appropriate services.
