Job summary

Corus Consultancy is hiring a community support worker to assist clients with mental health challenges in Islington and surrounding areas. You will work 37.5 hours per week, Monday to Friday, with some flexibility for outside hours.

Travel across Islington and home visits are required to support clients in their own homes. You will carry a small caseload, deliver person-centered keyworking, support housing and budgeting tasks, assist with appointments, and coordinate with partner services while

• Experience supporting people with mental health issues to live independently in the community.
• Understanding of approaches for issues like substance use, dual diagnosis, risk of homelessness, and domestic violence.
• Ability to help with budgeting, rent arrears, and housing options.
• Experience with client-centred support for vulnerable people with significant needs and risk.
• Ability to develop and deliver action plans with risk management and needs assessments.
• Ability to liaise with service providers to improve client services.
• Knowledge of safeguarding issues and risk assessments.
• Experience in a strength and recovery-based model to promote independence and inclusion.

Responsibilities

• Hold a small caseload of key clients and run person-centred keyworking sessions.
• Support clients to manage their diagnosis with holistic strategies addressing emotional, practical and housing needs.
• Support medication compliance of clients.
• Assist clients to attend statutory appointments for mental health treatment.
• Provide housing advice to maintain tenancies and explain rights and responsibilities.
• Help clients upskill in daily living to sustain accommodation, including bill and household management.
• Assist clients to access activities toward greater independence in the wider community.
• Collaborate with other departments and agencies to address client needs.
• Record all contacts accurately on the database in line with policy.
• Identify and report safeguarding concerns and incidents to minimise risk.
• Note: role involves lone working; travel across Islington area is required.

Job description

MUST HAVE Enchanced DBS (Child and Adult) and on the update service.

The successful candidate will be required to work 37.5 hours per week, working Monday - Friday. Core working hours are 10AM - 5PM, but flexibility is required to occasionally work outside of these hours. Temporary cover is required for approximately 2 months.

Please note: you will be required to travel across the Islington area supporting clients within the community and their own homes.

In this position, you will be required to;

• Hold a small caseload of key clients and carry out person-centered and psychologically informed key working sessions
• Support clients to manage their diagnosis by implementing holistic support strategies that also aim to address their emotional, practical and housing management needs
• Support medication compliance of clients
• Support clients to attend statutory appointments to comply with mental health treatment
• Provide housing advice to clients for them to maintain their tenancies including advising clients of their rights and responsibilities as well as advocating on their behalf if necessary
• Support clients to upskill regarding daily living skills, to enable them to sustain their accommodation independently, including bill management and household management
• Support clients to access activities which will assist them in working towards gaining greater independence within the wider community
• Work in partnership with other departments and external agencies to ensure that client needs are assessed and addressed appropriately
• Ensure all client contacts are recorded appropriately and accurately on the database and in line with policy and procedures.
• Minimise risks to clients by identifying, reporting and following up any safeguarding concerns and incidents.

To apply for this role, you must have;

• Experience supporting people with mental health issues to live independently in the community
• Experience and/or understanding of the range of approaches appropriate to working with issues such as substance use, mental and physical ill health, dual diagnosis, challenging behavior, risk of homelessness, offending, antisocial behavior, social isolation, domestic violence, gang-related issues, young parents, leaving care and learning disabilities.
• Understanding of how to support clients to carry out tasks such as budgeting, calculating benefit entitlements and understanding rent arrears and housing options.
• Experience of working in a client-centred support service working with vulnerable people presenting significant levels of need and risk
• The ability to devise and deliver on action plans through high quality risk management and needs assessment.
• The ability to effectively liaise with a range of service providers and agencies to establish or improve services for clients
• A good and current understanding of safeguarding issues and the ability to undertake comprehensive risk assessments.
• The ability to work within a strength and recovery-based model and engage and motivate people to move towards an appropriate level of independence and inclusion
