Job summary

Simon Community Scotland is seeking a dedicated Support Worker to empower people experiencing homelessness, addiction, and trauma. You will provide compassionate, person-centred support to help individuals secure safe housing and access health and welfare services.

Working within a community-based team in Glasgow, you'll follow a trauma-informed approach, build trusted relationships, and collaborate with local authorities and partner agencies to deliver coordinated care.

Qualifications

  • SVQ 3 social care or willingness to work towards qualification.
  • SSSC registered.
  • Ability to work within the parameters of the SSSC Code of Practice.
  • Trauma informed practice desirable.

Responsibilities

  • Create and manage personalised support plans for people, prioritising tasks and managing resources effectively to help them achieve their goals.
  • Monitor progress against established goals and adjust support plans as needed to ensure the best possible outcomes.
  • Maintain accurate and detailed records of your contacts using our Netsuite platform throughout your shift.
  • Work with partners to coordinate care and ensure safe housing access.
  • Provide emotional and practical support to individuals to manage challenges.
  • Support people to access benefits and a range of health and wellbeing services.
