Job summary

The Forward Trust is seeking a Substance Misuse Practitioner to join the team at HMP Chelmsford on a fixed-term 12-month contract. You will support prisoners with problematic substance misuse using harm reduction and abstinence-based approaches, building care plans and connecting clients to housing, healthcare and resettlement services.

You will work within an integrated healthcare team, delivering 1:1 and group sessions, maintaining case notes, and producing progress reports to support positive

Qualifications

• Compassionate, non-judgemental support for people affected by substance misuse.
• Experience with substance misuse or related support needs.
• Ability to complete effective assessments and develop care plans.
• Experience delivering structured 1:1 and/or group sessions.
• Strong written and interpersonal communication, able to work in a multidisciplinary team.

Responsibilities

• Manage a caseload of clients with substance misuse.
• Provide evidence-based support using harm reduction and abstinence approaches.
• Complete comprehensive assessments and care plans for each client.
• Deliver 1:1 and group sessions on substance misuse and mental health.
• Assist with transition and resettlement planning and liaise with external services.
