Job summary

You will work flexibly across locations as needed, delivering person-centered care and collaborating with the team to ensure wellbeing and safety.

You will follow care plans, document care activities, and engage with residents and their families to promote independence and connection to the community.

Qualifications

• Good English (written and verbal).
• Proficient written skills to maintain care plans and care records.
• Communication skills to build rapport with residents and inform colleagues.

Responsibilities

• Ensure residents are at the heart of care delivery and respect their wishes.
• Contribute to the efficient running of the service.
• Support residents to maintain relationships with their local community.
• Follow care plans and other supporting information regarding residents.
• Inform the Team Leader of any changes in a resident's needs.
• Promote and safeguard the welfare of those being supported.
