Job summary

Leicestershire Partnership NHS Trust in Hinckley is seeking a Community Hub Coordinator to support Community Nursing and Therapy teams. The role involves admin tasks, using SystmOne for patient records and referrals, and handling day-to-day hub operations.

You will work across various community settings with flexible shifts (8:00–18:00, including weekends/bank holidays) and a requirement for a full UK driving licence and access to a vehicle.

Excellent telephone manner and communication skills.

Responsibilities

Providing comprehensive administrative support to Community Nursing and Therapy teams.
Using the electronic patient record system (SystmOne) to manage patient records, referrals and clinical tasks.
Completing electronic referrals and processing incoming and outgoing tasks.
Undertaking accurate data entry and maintaining patient information records.
Assisting with the collection and reporting of audit data.
Supporting Community Nursing and Therapy staff, Team Leads and Clinicians in the delivery of high-quality services.
Ordering, monitoring and maintaining stock levels of equipment and supplies.
Responding to telephone enquiries and directing calls appropriately.
Supporting the effective day-to-day operation of the Community Hub.
