Integrated Care 24 Ltd. is recruiting Bank Advanced Clinical Practitioners to join the Primary Care team at Herstmonceux. You’ll assess, diagnose and treat patients with a wide range of conditions, prescribe medications, and work within an integrated system to improve access and outcomes.

Qualifikationen

• Registered with NMC, HCPC or equivalent registration.
• Advanced Clinical Practice qualification or equivalent training.
• Qualified Non-Medical Prescriber.
• Completed Physical Assessment of Adults qualification and Minor Illness training.
• Excellent clinical assessment, diagnostic and decision‑making skills.
• Confident managing patients autonomously within scope of practice.
• Strong communication and interpersonal skills.
• Committed to clinical governance and CPD.

Aufgaben

• Deliver advanced clinical care across undifferentiated and acute presentations.
• Prescribe medications independently following assessment.
• Manage complex patient presentations and determine discharge or referral pathways.
• Support long-term condition management and women's health needs.
• Contribute to quality and clinical governance initiatives.
• Collaborate with GPs, nurses, AHPs and wider partners for coordinated care.
• Support service development and innovation in primary care.
