Job summary

Mersey Care NHS Foundation Trust is seeking a Band 5 Mental Health Practitioner to join the Early Intervention Team for Psychosis Sefton. Qualifications

Responsibilities

• Coordinate care of service users through assessment, care planning, and evaluation.
• Ensure care plans meet assessed needs and refer to unqualified staff as per plan.
• Liaise with family, carers and significant others in assessments and care planning.
• Develop and monitor risk management plans with the team input.
• Direct unqualified staff to ensure high standard of care and documentation.
• Contribute to teaching sessions and bid for continuous professional development.
