Job summary

Pennine Care NHS Foundation Trust in Bury is hiring a full-time Early Intervention Team Care Coordinator (Band 5). Based at 3 Knowsley Place, you will manage a caseload of young people and adults with first-episode psychosis, work on a duty rota and support recovery through psychosocial interventions.

The role requires travel across the Bury footprint, active involvement with carers and families, and collaboration with the MDT under supervision of the Team Manager and Clinical Lead.

Qualifications

• Experience working with clients with serious mental illness within an Early Intervention Psychosis Team.
• Ability to work effectively in a community setting and as part of a multi-disciplinary team.
• Knowledge of safeguarding, risk assessment and care planning within NHS guidelines.

Responsibilities

• Overseeing a caseload of service users aged 14-65 with first-episode psychosis.
• Participate in a duty rota and contribute to service delivery across the borough.
• Support recovery through psychosocial interventions and care planning.
• Collaborate with carers, families and MDT to promote recovery goals.
• Signpost, refer and liaise with partner services as required.
• Travel across the Pennine Care footprint to fulfil duties.
