Pennine Care NHS Foundation Trust is offering a Secondment post for a full-time Band 5 Care Co-ordinator in Bury. The role supports patients with First Episode Psychosis and those at risk, travelling across the Bury footprint.

You will work with Registered Mental Health Nurses, Social Workers and Occupational Therapists, participate in a duty rota, and provide recovery-focused care with psychosocial interventions.

Responsibilities

• Overseeing a caseload of service users with first episode Psychosis aged 14-65 years.
• Travel across the Bury footprint as part of the role and contribute to the duty rota.
