Job summary

Stepping Stones to Independence is seeking a Community Support Worker to provide home-based care in the South Gloucestershire/Bristol area. You will follow the Care Plan, support service users with identified needs and ensure safety and accuracy in all documentation.

The role requires completing the Care Certificate before probation, using our care software, and liaising with social security and housing departments when needed. You will work in a client-centered, respectful environment.

Qualifications

• Must complete Care Certificate before probation.
• Understanding of the Mental Capacity Act (2004).
• Ability to complete and maintain accurate client records.

Responsibilities

• Visit service users in their homes and follow the care plan to minimise risks.
• Complete MAR and notes, report to senior when needed, using provided software.
• Log in/out using on-site device and maintain daily records.
• Assist with personal care and forms, liaise with Social Security and Housing Benefit as appropriate.
• Attend care plan meetings and contribute to person-centred planning.
• Respect privacy, dignity and confidentiality, report incidents promptly.
• Support users to make informed choices and remove barriers in daily living.
