Community Case Management Limited is seeking a kind, reliable Support Worker to assist a woman in her 30s with an acquired brain injury and her young daughter at their home in the United Kingdom. The role focuses on person-centred support, daily routines and enabling participation in community activities.
You will document shifts, follow strategies from professionals, and work as part of a team. A full driving licence is essential and DBS disclosure is required.
We are looking for a kind, reliable and proactive Support Worker to support a woman in her 30s with an acquired brain injury (ABI), who lives with her young daughter in their forever home. The role focuses on providing person-centred support with daily routines, including before and after school, as well as some full day shifts to support community, social and leisure activities. Our client experiences difficulties with memory and aspects of executive functioning, so you will help her to plan, organise and complete tasks using established strategies, rather than doing things for her. Support will also include budgeting and managing finances, maintaining social relationships, and promoting healthy eating and physical activity for both herself and her daughter.
You will work as part of a wider support team, following strategies and recommendations developed by the neuropsychologist and other professionals. We are looking for someone who is a keen learner, open to feedback and willing to develop their understanding of ABI and the individual strategies that support our client. Being able to work collaboratively and consistently as part of a team is essential, as is building a positive and respectful professional relationship with our client and her family.
Clear and accurate daily record keeping is an important part of the role. You will be expected to document what happened during each shift, the support provided, strategies used, progress made and any relevant observations or concerns. We are looking for someone who is patient, organised, observant and dependable, with a genuine interest in supporting independence and helping our client and her daughter maintain a happy, healthy and fulfilling family life.
This is a bank position Monday-Sunday between 7am and 7pm
Weekdays: £15.65 per hour
Weekends: £17.70 per hour

CCM provide direction, supervision and training to support workers so they feel confident and capable in their role.
