Job summary

Phoenix Futures is seeking a compassionate Family Support Worker for HMP Belmarsh to join the Release and Transfer Team. You will provide tailored interventions and recovery approaches for clients and their families within 12 weeks before release and 12 weeks after, aiming to support continuity of care and successful resettlement.

Based in a prison environment, you will assess needs, manage a caseload and facilitate referrals to community services, working with partners to reduce re-offending

Qualifications

  • Experience in health and social care, ideally within substance use, mental health or comparable field.
  • Committed to patient care and first-class service provision.
  • Evidence of effective partnership working and networking for client benefit.
  • Evidence of managing a busy caseload or tasks and excellent organisational skills.
  • Proficient in Microsoft Outlook, Word and Excel; familiar with database management.
  • Excellent written and verbal communication skills.
  • Understanding of complex issues and health priorities in custody.
  • Determined, with a drive to succeed and willingness to learn.
  • Passionate about making a real difference to lives supported.

Responsibilities

  • Provide tailored interventions and recovery approaches within 12 weeks prior to release and 12 weeks post release.
  • Carry out assessments in the prison and Leavers Lounge and support continuity of care.
  • Caseload management and onward referrals to families at risk.
  • Work with offenders to deliver care-planned support using evidence-based models.

Job description

We are seeking a compassionate and skilled Family Support Worker to join our Release and Transfer team at HMP Belmarsh, supporting positive change and making a lasting impact.

The Role

As a Family Support Worker, you will be part of the Release and Transfer Team providing tailored interventions and recovery approaches to clients and their families within 12 weeks prior to release and 12 weeks post release.

You will work in the prison and the Leavers Lounge carrying out assessments and assist with continuity of care for prison leavers. This includes accompanying people in treatment to appointments in the community after release.

You will case manage clients and provide pre-and-post-released advice, support and onward referral to families most at risk.

You will be required to work directly with offenders delivering effective care-planned support using appropriate models, interventions, and approaches tailored to meet the individual needs of the client group. Previous experience of working within health, rehabilitation, treatment, and re-offending prevention services would be beneficial.

About You

To join us as a Family Support worker at HMP Belmarsh, you will need:

  • Relevant experience of workingin health and social care, ideally within the field of problematic substance use, mental health oranother comparable field.
  • Committed to patient care and first-class service provision.
  • Evidence of effective partnership working and the ability to network for the benefit of theclient and service.
  • Evidence of managing a busy caseload or tasks and excellent organisational skills.
  • To be proficient in the use of Microsoft Outlook, Wordand Excel and befamiliar withdatabase management systems.
  • Excellent communication skills, both written andverbal.
  • Have an understanding of complex issues and health priorities presented by people entering custody.
  • Determined, with a drive to succeed and a willingness to learn.
  • Passionate and enthusiastic about making a real difference to the lives of people we support.

This role will be Monday - Friday (every other Friday off) with occasional Saturday working as part of a rota.
