Job summary

Two Family Solutions Practitioner posts with core hours primarily weekday afternoons/evenings, based in the Telford & Wrekin Council area. You will deliver targeted, intensive support to children and families to help stay together and reduce statutory intervention where possible.

Role involves home visits, collaboration with partners, and risk assessment with a focus on safeguarding, resilience, and sustainable change. Hybrid and flexible arrangements are offered within an Outstanding service.

Qualifications

• Experience delivering support to children, young people and families.
• Ability to build trusting relationships within diverse networks.
• Understanding of safeguarding principles and processes.
• Strong communication and partnership-working skills.

Responsibilities

• Deliver intensive support to children, young people and families.
• Help maintain children within their family and community networks.
• Work with families experiencing care entry risk or breakdown.
• Support reunification from foster care or residential settings.
• Complete strengths-based assessments and write evidence-based plans.
• Provide practical interventions to improve wellbeing, education and health.
• Collaborate with social care, Early Help and partner agencies.
• Maintain thorough case records and outcome-focused practice.
